Any individual who enters these Sweepstakes agrees to be bound by the terms, rules and conditions provided herein.

1. Eligibility. Entrant must be a legal resident of the United States and must be at least eighteen (18) years or older at the time of entry. Employees, officers, directors, contractors, subcontractors, and any other agents, and immediate family members of the same, of Winnie Community Hospital, LLC d/b/a Riceland Medical Center (“Sponsor”), its subsidiaries and affiliates are not eligible to enter. VOID OUTSIDE THE UNITED STATES AND WHERE PROHIBITED. This Sweepstakes is governed by the laws of the State of Texas; all applicable local, state, and federal laws, rules and regulations apply. “Immediate family members” shall mean spouses, siblings, parents, children, and “in-laws.”

2. How to Enter; Odds. NO PURCHASE NECESSARY. Submit an entry form. Limit one entry per person. By submitting an entry form, entrant provides his or her express consent to be contacted by Sponsor. The submission of more than one entry by the same individual shall void all entries. Sponsor shall not permit any automated entry devices or other mechanisms. Entries must be submitted during the period of __________ [e.g., Wednesday, October 3 to Friday, October 5, 2018] (“Entry Period”). Winner shall be selected in a random drawing from all eligible entries Sponsor receives during the Entry Period. Odds of winning depend on the number of eligible entries Sponsor receives.

3. Prize and Selection of Potential Winner. The prize to be awarded: one (1) YETI® Tundra® Haul Cooler. Total approximate retail value of prize is $399.99. Sponsor makes no warranties, express or implied, regarding the prize. Prize shall not be transferable, redeemable by potential winner for cash value, or substituted for another prize. Potential winner assumes all taxes, fees and surcharges on the prize, if any. Sponsor shall attempt to contact potential winner within five (5) days of the closing of the Entry Period by phone or e-mail. Potential winner shall then have five (5) days after making successful contact with Sponsor to claim the prize at Sponsor’s place of business, located at 538 Broadway Avenue, Winnie, TX 77665 between the hours of 8 am and 4 pm. (Central Standard Time) Monday-Friday, excluding holidays and weekends. If Sponsor cannot successfully contact the first potential winner, or if the first potential winner rejects the prize or does not comply with these terms, rules and conditions, the first potential winner shall forfeit the prize and Sponsor shall select an alternate winner from the remaining eligible entries.

4. Disclaimer of Sponsor for Entries; Disqualifications. Sponsor claims no responsibility for: (a) any and all inaccurate, incomplete, or otherwise incorrect information with respect to the entries; (b) any interruptions, failures, or defects of any telecommunications networks, utility providers, or computer systems used in this Sweepstakes; (c) identity theft; (d) the tampering, unauthorized use and access to, destruction of, or other tampering with entries; (e) and lost or misplaced entries. Sponsor reserves the right to disqualify any entrant(s) who provide false, misleading, fraudulent or otherwise deceptive information to participate in the Sweepstakes or who otherwise seek to undermine the integrity of the Sweepstakes.

5. Miscellaneous.

a. Indemnification. By participating, entrants and potential winner agree to release, discharge, covenant not to sue and indemnify Sponsor, its employees, officers, directors, contractors, subcontractors, and any other agents, and immediate family members of the same, as well as those of Sponsor’s subsidiaries and affiliates, from any and all liability for loss, harm, damage, injury, cost or expense of whatsoever, including without limitation personal injury or injury to property which may occur in connection with the Sweepstakes, or acceptance, possession, and/or use or misuse of the prize or participation in any and all activity related to the Sweepstakes, and for any claims based on rights of publicity, defamation, invasion of privacy and/or the delivery of the prize. Sponsor shall claim no responsibility if the Sweepstakes cannot occur, or cannot deliver the prize due to any and all acts of God, acts of war, civil unrest, acts of terrorism, natural disasters, or the weather.

b. Intellectual Property. All entries shall become the sole property of Sponsor.

c. Amendments. Sponsor reserves the right, in its sole and absolute discretion, to amend or otherwise supplement these terms, rules and conditions at any time without notice.
