Team Lead Chronic Care Management

Job Summary

Riceland Healthcare System is seeking an experienced and motivated Team Lead for Chronic Care Management (CCM) to oversee and coordinate daily operations of the CCM team. The ideal candidate will ensure quality patient engagement, compliance with CMS guidelines, and smooth execution of care coordination activities that support patients with chronic conditions.
The Team Lead will serve as a liaison between the CCM team, providers, and administration to improve patient outcomes and operational efficiency.
Key Responsibilities

  • Supervise and support the CCM team, ensuring all patient calls, follow-ups, and documentation are completed timely and accurately.
  • Oversee the development and maintenance of care plans for patients with chronic conditions.
  • Ensure compliance with CMS guidelines and organizational protocols for chronic care management.
  • Monitor team performance metrics, track monthly targets, and provide feedback and coaching to staff.
  • Coordinate with physicians, nurses, and clinical teams to ensure continuity of care and effective communication.
  • Conduct audits of care plans and documentation to maintain quality and compliance standards.
  • Identify workflow gaps and implement process improvements for efficiency.
  • Provide training to new CCM staff and ensure consistent adherence to policies.
  • Generate periodic reports for leadership on patient engagement, performance, and billing readiness.

Qualifications

  • Education: Bachelor’s degree in Nursing, Healthcare Administration, or a related field (or equivalent experience).
  • Experience:
  • Minimum 3–5 years of experience in Chronic Care Management, care coordination, or population health.
  • 1–2 years of supervisory or team lead experience preferred.
  • Skills & Knowledge:
  • Strong understanding of CCM, RPM, and Transitional Care Management (TCM) guidelines.
  • Proficiency in eClinicalWorks (ECW) or similar EMR systems.
  • Excellent leadership, communication, and problem-solving skills.
  • Detail-oriented with strong organizational and multitasking abilities.

Job Type: Full-time
Pay: $55,000.00 – $75,000.00 per year
